Which are the Guaranteed Combos?

Which are the requirements of a combo to be guaranteed?

In the training mod I set:
COM Reaction - Guard - Guard All
COM Options - Recovery - On
COM Options - Stagger Escape - Fastest

But I'm not sure the combos that work with this combination are guaranteed. For example, if I am too far from the opponent, the first attack might lands, but then I can't connect.
